Botox vs. Dysport — Which Is Right for You? A Mission Viejo Physician's Perspective
One of the most common questions I hear from patients in my Mission Viejo practice is: "Should I get Botox or Dysport?" It's a great question, and the honest answer is — it depends. Both are excellent products, and I use both regularly. Let me walk you through the real differences so you can have an informed conversation with your provider.
What Botox and Dysport Have in Common
First, the similarities. Both Botox (onabotulinumtoxinA) and Dysport (abobotulinumtoxinA) are injectable neurotoxins derived from botulinum toxin type A. They work the same way: by temporarily relaxing the muscles that cause dynamic wrinkles — the lines that form when you frown, squint, or raise your eyebrows.
Both are FDA-approved, both are extremely safe when administered by a qualified provider, and both produce beautiful, natural-looking results. The differences are more nuanced than most marketing would have you believe.
How They Differ: Onset and Spread
The most noticeable clinical difference is in how the products diffuse after injection. Dysport tends to spread a bit more than Botox, which means it can cover a larger area from a single injection point. This can be an advantage in broad, flat areas like the forehead — and something to be more careful with around the delicate muscles near the eyes.
Dysport also tends to kick in slightly faster. Many patients notice results within 2 to 3 days with Dysport, compared to 4 to 7 days with Botox. If you have an event coming up and want quicker onset, Dysport can be a smart choice.
Duration: How Long Do They Last?
In my clinical experience, both products typically last 3 to 4 months for most patients. Some patients report that one lasts slightly longer than the other for them personally, but the scientific literature shows comparable duration. Individual variation matters more than the product itself when it comes to longevity.
Factors that affect duration include your metabolism, the treatment area, and how active those muscles are in your daily expressions. Over time, with consistent treatment, many patients find they can extend the interval between sessions.
The Unit Question: Why Dysport Uses More Units
This is where confusion often arises. Dysport units and Botox units are not interchangeable. The conversion is roughly 2.5 to 3 Dysport units for every 1 Botox unit. So if someone tells you they got "60 units of Dysport," that's roughly equivalent to 20-24 units of Botox.

So Which One Do I Recommend?
In my practice, I choose between Botox and Dysport based on the individual patient and the treatment area. There's no universally "better" product. Here's my general approach:
- Forehead lines: Dysport can work beautifully here because of its natural spread across the broad frontalis muscle.
- Frown lines (the "11s"): Either product works well. I'll discuss your anatomy and preferences.
- Crow's feet: Both are excellent. Precision matters more than product choice here.
Some patients develop a mild preference for one over the other after trying both. I'm happy to accommodate that. The most important factor isn't which product you choose — it's who is injecting it and how well they understand facial anatomy.
The Provider Matters More Than the Product
I want to be direct about this: the skill and experience of your injector matters far more than whether you choose Botox or Dysport. A beautifully placed injection with either product will give you a natural, refreshed look. A poorly placed injection with either product can lead to heaviness, asymmetry, or that frozen look no one wants.
